Should the ,,always an obsession is present" mechanic also apply to totems?
Basically one totem will always light up with no effect from the very start. When cleansing that totem nothing happens. If the killer brings ruin for example then there would be 2 lit totems in the trial. If a killer brings 4 hex perks that would light up 5 totems then of course these 5 totems are only occupied by the perks you carry.
Would add a bit of guessing if the killer used a hex or not, it would make hex:retribution slighty better and also drag out the game a tiny little bit.

Survivors (for the most part) will not know whether or not a hex perk is in play at the start of the match. Before this change, killers would know whether or not someone had DS the second that they loaded in.

This isn't a bad idea. Honestly, I'm of the mindset that all totems should be lit up:
1). It encourages survivors to spend more time doing them, even if the killer didn't actually bring hexes.
2). It gives survivors more bloodpoints for breaking them.
3). The killers can still know where their totems actually are like they do now.

Will a 6th Hex Totem appear with nothing on it?

Uh no as I said when a killer brings up 4 hex perks that light up 5 totems then 5 totems will glow with the effect of the 4 perks.. so basically that ,,no effect hex" is not existing in this case.
This idea is there to make one survivor stop for a few seconds doing this ,,pointless" totem + a team will never know if the killer is playing now with 3 perks or still 4 (also I mentioned that slight retribution buff since it can now trigger twice potentially)

Extra 1500 BP each game? I'd be good with that. Would also make survivors second guess if they actually removed a hex perk or not making it more difficult to figure out the killers load out quickly.

Survivors already are guessing if the killer is using a hex. It's called noed and requires survivors to cleanse all dulls (not just one totem) on the map or get hit by it.

That's literally just Undying, you want to make Undying basekit... which honestly might not be an awful idea given how Hex perks are all or nothing. But then Undying would have to do something else or something more.
