Can someone please explain how this didn't injure him?
Not entirely sure if due to latency, but i've had this happen several times before too. You can even see the blood appear when the hatchet hits him, but for some reason it didn't injure him?

I guess for the same reason people get hit when the hatchet is 10 metres away from them.

My assumption is the same as yours Botiz. All I can think of is latency especially after the video the devs showed of examples of latency occuring for both the survivor end and the killers end. If you would have got bloodpoints I'd say different but since the little bloodpoint indicator never popped up on the left I'm guessing it's latency since I've had somewhat similar situations happen to me in the past when using the Huntress.

My guess is that it was latency. He dropped the pallet on his end before you threw the hatchet, the action reached the dedi first and therefore it registered the hit on the pallet not on him.
Dumb but that's probably how that went down.

In a certain sense, you did hit him, you can see the blood spattering. I can only guess the hit wasn't eventually validated because on his screen the pallet was already down and you hit that instead. Although, that's in contrast with what the devs say about hits being client sided.
I've had the similar scenario play out when I threw a hatchet to a Claudette who dead harded. I saw the blood and I could swear I timed it perfectly to bait and it landed after the animation, but I wasn't given the hit. Which is doubly weird considering dead hard basically never works.
So, desync/latency or possibly a hack.
