I came across an interestingly short game where I got hooked ONCE and I was already in the struggle phase. Is there an addon or offering that does that? If not, then I believe they might have been cheating

    No, there is no perk like that. The closest thing is monstrous shrine but that only speeds it up and would not put you in the second stage instantly.

    You should say which killer it was because, and I know this is obvious, if it was pyramid head and you were caged that would put you onto second hook instantly.

    EDIT Tatariu must have been typing the same time as me so their post didn't show which is why it mostly says the same thing :p

    It was a deathslinger. Nothing seemed fishy about him the whole game until I started noticing people dying too quickly on the hook. I didn't even see them try to free themselves, they just immediately went into the struggle state.
