For me I think it's Clown's Pulcinella cosmetic, there's just soemthing about it that's so so unsettling, same for his santa one with the maggots squirming in his belly

A close second is Quinton also

    Creepiest in terms of just like a general sense in terms of how it looks probably has to go to Clown’s Pulcinella. Some runner ups for it could be bunny legion or hag’s or doctor’s hallowed blight cosmetics since both of theirs are possibly creepiest of the hallowed blight sets.


    Creepy in my terms not only has that sense of dread but some gore on top of it giving a reason to fear on top of general creepiness and that’s why according to my opinion I think pyramid head’s paid cosmetic is scary and creepy all together. On top of the dark colors his helmet and the mysteries behind it make him creepy as is but with black and those veins ups it up a bit more. On top of that the barbed wire physically being on and in him even so much as to show his muscles and bone being revealed just gives me that extra fear considering if that’s just what’s on him what could he dish out to punish others.

    Honestly? Base Doctor. His design is peak horror imo and he always gives me the chills. Pretty sure nothing beats it for me.

    The Hag cosmetic where she has empty eye sockets! Last time I played against a Hag with that cosmetic I wanted out of the match because I can't stand looking into empty eye sockets
