5 active hex totems in one round?

So me and a friend were in a match against a Mike who was using 2 hex perks however in game there was 5 active hex totems.
Can anyone explain this or should we suspect foul play?

it would be helpful if you could tell us which two hex perks he had

He probably used Hex: Undying together with another Hex-Perk. Undying makes it that a cleansed Hex-Totem changes to a Dull-Totem. Which means, if you cleanse the other Hex, it will still be active. If you are unlucky, you have to cleanse it 4 times.

Do you remember the perks they had? They could have had Undying+ other Hex perk and you just got unlucky.
