What happens to killers after a failed trial?
So when survivors die they are either sacrificed or get a mori, but what happens to a killer if all survivors escape?

Whether or not survivors escape or die doesn't matter in the slightest. It is whether or not The Entity gets what it wants...all those emotions of dread, hope, fear, etc. Killers are there to ensure survivors keep producing those emotions. Killers who fail or refuse to do this are, I believe, tortured. There are exceptions though. Like Pyramid Head, for example. The Entity has zero control over it.

What do you think 'Entity Displeased' means?
I like to think that if a killer doesn't do their job, they get punished, being mentally tortured to drive them into doing better.
and if a kill constantly refusing or failing at their job, then they are either turned into a survivor or sent to the Void.
this isn't canon btw, this is how I think what happens, I really don't know what happens if a killer fails at meeting the requirements in the trials

I seem to remember reading something in the lore about there's a place where 'broken' survivors go when they're not capable of producing the desired tasty emotions anymore. I wonder if the Evil Sky Crab tosses killers there too.

yeah, that's the Void.
if they constantly fail to make sacrifices, then possibly.

I also wouldnt be surprised if Ghostface never really gets punished either. Whilst he might have failed, lorewise, he is trying. Torturing him wouldn't help a bit nor would it help the entity.
I can just imagine the entity whacking Demogorgon with a roll of newspaper though. "Bad demodog, go sit in the corner." *whine*
