The game map mistake
If you watched all the Saw movies you will know that the person in the picture is Doctor Gordon. But he survived the bathroom game. How did he still die there?
And his foot is still in the other corner that means he escaped right?
I think the devs confused him with Mark Hoffman cuz he was the one who has been locked in the series finale.

If the realms are composed of the memories of a killer, then it would stand that not everything would be accurate in a realm. The developers also said they always wanted the realms to feel off in some way, as if The Entity is trying to recreate a location perfectly but can't do so. Or maybe the developers just screwed up, I don't know.

It could be both. They screwed up but their screw up fits with the lore they try to push.
