It depends on the situation, and what exactly you're after.
You said the three ran into the exit area, but didn't exit. Why didn't you force them out? Hitting a survivor at the exit will nearly guarantee they run out the door and not risk going for the save. At the least, you could have forced one or two of them out, severely limiting their ability to coordinate body-blocks and saving the hooked survivor.
You also said you waited for the attempted unhook so you could make a grab, but you were being body-blocked from making the grab, so why did you wait? If you can't make the grab, make the hit. In the worst case you would have been chasing three people to the exit, but two would have been injured instead of just one, and you would likely have been able to get another down before they made it out.
You phased to the exit gate after it was open. Why? If you were trying to secure your one kill and you aren't going to force them out, why waist your cool down and risk them walking right past you to the hook without you knowing they're there? If you'd stayed at the hook, the survivor up there would probably signal he was being camped and the other three would have left instead of going back, or he may have tried for the Kobe and severely limited his time on the hook.
Lastly, if you're having trouble with the mid-game with all four survivors making it to the end game, design your build for end-game advantage. Running NOED, Blood Warden and Remember Me could all have made the difference for you.
Short of that, your best option might have been using your phase to get ahead of the running survivors, knowing they're blocking you from reaching the injured survivor from behind, and gotten the hit there. I guess it depends on how much time you had.

They feel like a 3 man swf there coordination was off the charts through out the trial and I had a gut feeling as soon as I hit one at lest one would of booked it for the save

There was about a 10 second period where they tried to bait me to hit. The one got to a spot where I couldn't go around her the wall was to close.

Honestly I feel like I panicked and wasted it as I could of phase back to the hook now that I think about it

I did have blood warden and I made the hook to early. I just started using it and forget I have it from time to time lol.
