As you can see on the image the Leather Grip add-on for the flash light isn't connected at all. Sadly I only notice after it got block so couldn't verify if it was impossible to get it but I assume it was impossible due to how the Blood Web work.
Did anyone had this issue?

That’s not broken any more than grip in the middle left or the rare stamp in the bottom left.
It just got entity blocked and you missed it doing it it seems to me.
