Survivor's Grunts of Pain while Injured are Increased by 40%/45%/50% and their Regular Breathing is Increased by 25%.
Every 40-Seconds of a Survivor being Injured will cause the Survivor to Begin to Cough for 2-Seconds. Survivors within 16-Meters of Another Survivor being Injured or Put Into the Dying State will Begin to Cough for 4-Seconds and once for Every-Other Time another First comes within this Range from an Injured or Dying Survivor.

Stridor: The Perk is a bit more Situational with Recent Changes.The Difficulty of the Game spikes Quite a Bit without Hearing Properly especially now with the not-too Uncommon use of Iron Will and other sound-Related Issues. This can retain its use-cases as Short-Range Indicator if paying Attention and allowing the Perk to have More Appeal and beneficiary to other Characters on the Roster.

The cough effect would be cool.
Take Plague's Audio for coughing and use that for the sound that occurs around every 40s.
It gives an in-game tell that the Killer is using Stridor as well as allowing for a specific Audio cue for the Killer to listen out for that can also bypass Iron Will.
Bonus points if the cough creates a visible breath cloud as to pay homage to the Perk Icon.
