IMO out of the ones you commented that is... I would have to say Adrenaline. I think it is a situational perk yes but the pay off for it is overwhelmingly big.
I play both survivor and killer pretty equally. I rarely use adrenaline as survivor. But when I go up against it as killer. Those clutch moments at the end game kill me every time. When I am about to secure a hook by picking up a survivor and take them to the hook gen 5 pops and boom they're back up and running again.
Don't get me wrong I like the perk. I just think it needs a secondary draw back.

Sprint Burst. None of the others are OP.
Edit: to make it clear, I don't think it needs a nerf. It's actually game balancing considering gen spawns in dead zones.

Just to clarify I don't it's an OP perk. But dead DH through a trap should ideally not be a thing, or when getting directly hit by a hatchet.

As @glitchboi stated, Unbreakable. Either that or BT, when used in aggressive SWFS. Iron Will is a bit 'there' under the right circumstances.
And it's not that Unbreakable is necessarily 'broken', it's just ridiculously powerful.

If i could select both DH and SB i would. DH isn't "op" per se, it just flies an inch under the radar of complete bull. The amount of ez loops you can string together with it is disgusting, and it can turn a killers mindgame into a mindfail with a single press of a button. SB is undeniably OP because in good hands you literally never have to worry about a wasted chase. If you always SB away early and proceed to hold W, you'll gain so much distance that for any non mobility killers it will be a huge waste of time for them to even bother chasing you. if this community applied the same level of scrutiny they do to ruindying, they'd be up in arms about SB and DH.

DH feels less fair IMO but SB is objectively more powerful by miles you end chases before they even start.

I have no issue with any of them, but in terms of a perk which is powerful without any conditions attached, Adrenaline is the big one. Dead Hard can be outplayed, and a good Dead Hard can make some distance, whilst a bad one will cost the survivor. Lithe has a cooldown. Decisive Strike is punishment for a killer who tunnels, and Head-On is a tactical perk which can be messed up if the killer spots someone enter a locker and gets there before Head-On charges.
Adrenaline gives a free health state and Sprint Burst, regardless of how badly they played, and is even granted when unhooked.

Pharmacy clearly.
Not a single other perk gives you a 100% guaranteed green medkit on your first search. It's so fast that Blight had to retire, when he saw its speed. It's so green that it makes you blend with grass better than Trapper's traps ever could. And the unlocking speed MY GOD! The last time Plague tried to drink from the fountain as fast - she choked on water and was hospitalized by the Entity's Ambulance.

Iron Will is probably the most OP one. As currently Dead Hard is buggued on console. If it wasn't i would say there would tied in 1rst place.
