Evil Incarnation
So the requirement was to kill all survivors in evil within 3, I did that using both judiths tombstone and tuft of hair and a last person mori, but mori-ing the last person using the mori offering didn't unlock the achievement, What am I doing wrong?

You need to kill them with the Tombstone.

All kills need to be from the standing position. This, because the mori kill and the add-on kill are technically distinct from one another. The achievement wants add-on kills, not mories.

Can't use mori. You have to grab them with tombstone.

this is what it says "In a public match, kill all 4 survivors with the TIER III Evil Within."
Nothing about using an add-on or offering

The wording doesn't state it, but it is how it's done. There's a reason it's among the most notorious achievements in the game.

Technically, it does.
"With the TIER III Evil Within"
Meaning it must be done with his power, not with anything else. The only way to do this is to use Tombstone Piece or Judith's Tombstone, and combining both Ultra Rare add-ons (ie. Fragrant Tuft of Hair + Judith's Tombstone) is overall substantially easier to do.

Yeah, and when the challenge says "kill by any means" bleedouts don't count. That is the issue, you need to know what the challenge actually wants and not what it says it wants. They LIE!
