Instant Down while being fully helaed without any "exposed" Perks/Powers

It happened 2 times in a row now. When I save someone from the hook,fully healed. No exposed status effect ! I go down instantly. The Killers didn't use their power they basic attacked me. First time it was a nemesis and 2nd game a plague. I just cleansed and went for the save and immediately I went down after just one hit. I checked the killers perks after the game and they didn't bring any perks with the exposed status effect and they were on console so they can't be hacking.
