I just had a match where this guy refused a heal to the point of fast vaulting to lure the killer to me and another teammate. I personally feel this falls under "working with the killer" but what would you call this?

Could be or could be new to the game. Playing as killer I am seeing many players that appear to be learning the game. I have been backing off and letting them escape if they seen to be be having substantial difficulty. Still chasing them and hitting them however.

It's been a few year however I still remember playing as survivor and not understanding some things. Especially how to remove a reverse bear trap, how it activates and so forth.

"working with the killer" would mean that both players were actively working together to get you killed and the killer not touch the person who was working with them.

From the description that you gave, it's very hard to tell. There are a few reasons for refusing a heal - for example, I regularly run Spine Chill & Resilience, it's incredible how many players get off gens and try to heal someone instead of focusing on the gen, even though I'm clearly working on that gen. I've normally 99'd my heal already so would tap the heal if I needed it in a chase for example, but that extra vault speed I get with Spine Chill means I can avoid doing that until I'm about to get hit.

Well this person in particular had no perks to benefit from this and actively gave away their team members positions. I'm certain that they were being a deadweight on purpose.

You have some people who take the game way to serious and get angry when you don´t play "good" enough for them for example.

A lot simply dc. But some want to rage.

I came to a rescue too late and he got to 2nd stage. He was clearly angry and clicked the whole time with his flashlight to me.
