Post match Survey Blood Points.
One of the clarifications of the post match survey is to enter how many blood points you recieved after the match. Is that the score on the last page or the total blood points I got including multipliers from offerings?

It's the score you receive on the first page of the end game screen summary (the bp with the multipliers). This means it can be above the 32K if you have offerings/perks.
You can put the value down that is shown on the first slide of the end game screen summary. Mandy said they are aware of the visual bug with BBQ, so they'll take this into account and you don't have to worry about doing the math.

I was wondering the same, since inputting the wrong number would void my entry,. I mean I guess I could enter it twice, once with each ammount of points, just to be sure, but I would rather do it right the first time.

I have not but, I can't find the source of the original survey but I'm pretty sure it's on for a few more days and said we will get the reward after the survey is over.

I found it.
"Simply play 7 matches and fill out the survey after each match to let us know how things went. Completing the survey for each match will earn you 1,000 Iridescent Shards and 300,000 Bloodpoints, to be delivered within a few days of the survey ending.
This survey closes June 6th at 1PM ET."
So you're probaly looking at around a week at least until your rewards come in.
