The killer has no TR when undetectable so there would be no heart on your screen showing you are in the TR.
When killers like the pig aren't undetectable, the heart TR will show.

Fun fact it does counter sadakos "lullaby"
Sadako cannot stealth if you have this enabled.
BHVR really has to fix that.
