Is there a bug where the hatch just... doesn't spawn?

I was trying to give hatch to a Sable who did a little happy dance and gave me her medkit, and I swear we looked all around the map looking for the hatch for the better part of five minutes and couldn't find it. Eventually, she just ran to a hook and pointed at it so we could get on with our days.

This isn't the first time this has happened, either.
