Several possibilities:
- BHVR does not want them in the game.
- The license Holders dont want them to be in the game.
- Both parties want them in the game, but they did not find a common ground in negotiations so far.
It will most likely be the last point and then nobody (outside of the people who are making the deals) know what is missing to come to an agreement.

We'll be getting our first 2 survivors no killer chapter soon, so that does open the door for more in that style. Supernatural seems like one of the more consistent requests over the years, so I could totally see them going for it.

This actually brings another point to the table - the right timing for a release. Now with Springtrap being a Killer only-Chapter, it makes sense to bring two licensed Survivors in a Midchapter.
But it is otherwise difficult to find a good spot where two Survivors can release at once.

Just adding, although probably falls under negotiations, discussions on getting Jared/Jensen approval for likeness in the game and possibly getting them in for voicelines could be issue. How much is adding Sam/Dean worth it for Supernatural fans if Jared/Jensen aren't involved and they can't use their likeness?

Supernatural license is owned by Warner Bros right? I think Nightmare remake is the only other WB license in DBD and WB also owns the Conjuring franchise which is the highest grossing horror franchise and it's kinda weird that's not in DBD since it's supposed to be a hall of fame of horror
so maybe WB is hard to work with in general
