I'd say distressing is a good start. many people will like to dictate what perks to use but honestly doctor is incredibly flexible build wise, and distressing just outright buffs him no matter what
ruins always good, unnerving presence (trapper perk) and coulrophobia (clown perk) can be good.
iron maiden (legion perk) denies his one counter to his terror radius which is getting in lockers,
try pairing any add-on "series" together, because most of their effects stack, the range add-ons are always good (electrode series) because having that extra shock range to deny unhooks, pallets, and vaults is always golden.
try looking up brickys video on the doctor, coming from one doctor main to another (while it may not be the most up to date video) it goes hugely indepth on all of his addons and all the perks that are incredibly useful on him. monto is another good source for build ideas from impossible skillchecks doctor, to stealth doctor (yes you can actually turn doctor into a stealthy killer! etc etc
if possible try getting into a custom game with friends, and practice using your shock to deny survivors using pallets and windows :) as well as learn how to swap inbetween treatment and punishment mode and when to fake it.
try to start of every match by being in punishment and swapping to treatment when you believe survivors might be near, (unless using omega terror radius build, in which case just go treatment ;) )
Hope i helped!

I think the best way to get good with him is to learn the timing to stop vaults and pallet drops. Most will just chase in Treatment Mode and shock people, wasting a ton of time, but with the right timing of switching form and shocking you can "ignore" many loops and be very powerful in chases.
Addons are very varied, they're all good and it's basically just a matter of preference and playstyle. Personally I love the Calm series because it can mess with survivors' perceptions and lead them to make mistakes, and the Interview Tape to get the increased range without the time penalty (but it requires more precision).
As for perks, again, it's personal. Imo TR perks (except Monitor and Abuse, which synergises well with the Calm addon series) are overrated and become basically useless after rank 15, since they basically enhance a passive playstyle . A perk I would suggest though is Save The Best For Last: aside from being a great perk in general, the Doctor can use it well because he can make the obsession inoffensive just by shocking them and putting them in Madness 3, without wasting stacks. Plus, with a good amount of tokens you can stop unhooks from afar with a shock and then hit-and-grab the unhooker, which makes runshed unhooks very dangerous against him.

I recommend against any of the range addons other than the common one. Shocking denies unhooks, but if you're using the long range long charge time addons, they still have time to unhook between charges.
Impossible Skill checks is my favorite build. Ruin, Unnerving presence, distressing, BBQ
Some people like to run lullaby instead of bbq, but double hex perks is super risky.
For addons, I generally go Calm and the beam tape. Beam is useful for sniping long range unhooks, gens, etc.
For a weird build, try:
BBQ, Insidious, Ruin, Iron Maiden
Addons: Calm, Restraint
Try to get madness up as quick as possible so that everyone starts seeing illusionary doctors, then stand near objectives using insidious to remove your terror radius. Survivors will think you're an illusion. I've grabbed more than a few survivors off gens this way.

Distressing is a must on Doctor, it expands TR and if in treatment, means you drive them crazy faster and more often.
Using the 3 gen strategy is very easy 4k unless you're going against smart and very skilled survivors.

highly suggest you focusing on my topic about Doctor playstyle
His therapy is not what only driving survivors to crazy

We need a stupid answer....
Go into treatment mode and exists. PROFIT guaranteed.

Distressing is one of the best perks for him. At the start of the match go into Treatment mode and let the huge terror radius do the job. Look at your add-ons, all are very useful except the one that makes your shock a ring. Iridescent King is a pain to deal with as survivor.
If you time your shock right you might get a free hit, once a survivor is shocked they won't be able to drop pallets and vault.
Or you can try the meme build of the sneaky Doc. Monitor & Abuse + the Calm series purple add-on and never go in treatment mode, only punishment. Sneak up on those survivors and grab them off the gen.

bbq, ruin, enduring, spirit fury is ur standard m1 killer build. dont respect pallets, don't waste too much time shocking people. your best addons vs good survivors are the ones that cause your redlight to always be on their back (super annoying to be against as survivor) because they won't be able to watch for your stain as u chase them.

Run the Game offering with TR perks

I play doctor like an a**hole.
distressing 3 for almost 30% larger terror radius plus 100% more deviousness points.
hex ruin to slow the game down - if only for a moment
Unnerving presence (increased skill check chance etc to survivors in your radius) - this can Be swapped out for overcharge if you like to focus gens.
bbq and chili - mostly for that 100% BP bonus stack on top of the deviousness stack. But also helps locate survivors.

Practice shocking survivors a few seconds before they get to a vault or shock the vault or pallet before they reach it. When they get shocked they won't be able to vault or drop the pallet and will be a free hit.
