    The entity had an oversight when creating a realm and did not think the survivors/killers would find a way to break their creations.

    The Entity is ineffable, not infallible. Her simulacra of our world's features are imperfect, mashed together from memory and sensation rather than reason, from minds and experiences in her stolen akashic records reflecting humanity without supernatural speeds or auras. Naturally, there will be errors in design.
