Decisive Strike Not Working? Sorry if this has been brought up before.

Was in a match and got grabbed off a gen by the nurse while DS timer was running. (Maybe 10 seconds after an unhook). Nurse grabbed me, and I hit my skill check dead in the center but the nurse didn't drop me, and I then died on a re-hook.
Is this a DS issue? Dedicated issue? Grabbing issue?
Sadly I was the killers only kill. The killer had mentioned after that the "Grab" didn't count for her... just the same as the DS didn't seem to count for me?
Thoughts?

This exact scenario happened to me just now! Nurse gen grabbed me, landed DS, it didn't free me and it deactivated DS for the rest of the match.

This happened before dedicated servers but was rare. Not sure what causes it.
