How do you get into the Entity's realm?
Some characters were taken by the entity at their death such as Amanda Young (Piggy). But alot of other characters didnt necessarily die they just got lost in the woods or something such as the Legion crew. Some characters like Wraith really have no explanation. So do you not have to be dead to be taken? In Kate's bio the entity basically attacked her as she played her guitar in the woods. Was she already dead? Maybe this is what happened to some of the other characters as well. Are these woods that are in various character bios the same forest? Also what decides whether your a survivor or a killer? Wraith is a killer but he wasnt a bad dude. And, correct me if I'm wrong, but didnt the Alchemist archives mention that he had played both roles?

The Entity chooses you or you choose you The Entity.
It can take the souls of those who die into it's realm or it can take them physically if their is a thin enough barrier between it's world and theirs.
As for how it chooses roles, there's no way of knowing.
Adiris was made a Killer when she begged the Entity to save other people out of hope and despair while Quentin was made a Survivor when he called upon the Entity to kill Freddy out of hate and bloodlust.
Like as not, the Entity just picks who it wants for a given role. It doesn't necessarily need a system. Why should it follow rules?
As for the Alchemist being both, the game and gameplay don't equate to the lore.
There are many more Killers and Survivors than those we're able to play as, some of them may switch roles on a dime. The playable roster merely represent those that are the most dedicated and exemplary of that role. The Entity might not want or allow one of it's best Killers to become a Survivor for a brief period of time.
Or it's entirely possible the playable cast do switch around too in the lore, but don't in game for the sake of making the game easier to understand. A new player would probably be confused if Evan MacMillan was killing them one game and helping them do gens the next.
Or perhaps that was just part of the Alchemist's ability to leave the Entity's Realm. If you've read his lore you know that he went outside the trials for a period of time. If he left and re-entered periodically in the past he might have had free reign to choose which role he wished to 'play' the Entity's games as.

I believe most survivors died, but they did not see it coming, like those people who got lost in the wood as you said. However, we know Quentin asked to be taken along with Freddy by some entity so he wouldn't be able to hurt his friends, if i recall correctly.

There seems to be many ways to enter it
For killers it's mostly a place full of death that creates some kind of hole that the entity can use to take people.
Once a hole is opened it doesn't close and that's how i believe a lot of survivors got taken.
For example Benedict visited the Macmillan estate and after studying it got taken.
We don't really know how exactly the entity choose the survivors she takes.

I think the Entity exploits a trait of each survivor's/killer's personality or will to get them in. Use Rin's vengeance to goad her into killing her dad, make Claudette's curiosity of plants get the better of her to get her in the woods, etc. As long as it can find some entertainment out of the people it wants to take, it'll find a way to pull it in.

Now I'm just thinking of a 4 man team of killers being chased by a Feng Min killer who has a little butter knife.... also has the Freddy thing been confirmed? That's what happened with those two?

Hey it could happen. I'd run away from a Feng brandishing a butter knife.
As for Quentin, from his lore:
"[Quentin] devoured all the information he could, on shared dream worlds, lucid dreaming, and the methods to control the dream space. Forcing himself to stay awake... ...he searched through dusty volumes, finding myths about the demons that live in dreams, trapping their victims in limbo and feeding off their terror."
IMO Quentin learned about and studied the Entity while trying to understand Freddy. Like Hag, he learned the true nature of the Entity and possibly how to call upon it, if even only subconsciously.
After Quentin's plan to defeat Freddy by manipulating the Dream World against him fails this happens:
"[Quentin] wished it had been him, not his father, who threw the gas can that ended Krueger’s life, that it had been him who cut Freddy’s throat. Perhaps that desire would be enough? This was a realm of the mind after all. He let it consume him, concentrating all his thoughts on wishing Freddy gone. His vision was obscured with roiling tendrils of fog and, when it cleared, he was somewhere else."
To me it sounds exactly like what happened to Hag, Plague, and Spirit. Except they became Killers, while Quentin became a Survivor.
And in case you have any doubts, Freddy's POV confirms that The Entity grabs him when Quentin wishes him gone.
"Slowly Freddy closed in on the boy. His fear was so strong now that Freddy could almost taste it, but his eyes burned with a defiant hatred that was almost admirable. Freddy drew back his claws. Then Freddy felt another presence with him; something old, powerful and dark. A miasma enveloped him and the only sensation was a sound like wooden beams flexing and creaking in the distance. The echoing groan of metal crushed against metal. Something arcane and unknowable, half-way between language and pure terror."
