The Entity's only prerequisites for the killers it takes up are 1. They must be willing to serve or easily broken 2. Must be or have great potential to be a killer 3. Come from a place of 'great tragedy' (the Entity is drawn to such places)

The Plague is the real killer, not Adiris. If Adiris was the true killer, the killer name should be something like 'The Priestess'. Adiris merely wields the Plague as a weapon. The Entity was drawn to ancient Babylon due to the mass deaths caused by the Plague and chose Adiris to represent it in the Fog because she is strong and highly devoted to the Entity, believing it to be her god. So in summary, Adiris herself did not kill in her life and she tried to do what was right, but due to the Entity's deception and the disease she has she met all the criteria for a good servant.

Adiris always had devotion for the Gods, it was very strong with her. When Adiris was in the cave surrounded by her dead followers and ready to accept her fate, her prayer to her Gods was sensed by the Entity and pretended to be one of her Gods. Now, she is entirely devoted to the Entity since she believes she is her God.
