Infectous fright is a good perk of her if you don't have many perks yet but I still like to use it anyway. If you use something like either deerstalker or nurse's you can use two perks to slug someone momentarily if someone happens to be close. Both allow me to see where the downed survivor is if they're either down or just as they get picked up. Piggy's generator monitoring perk can sometimes be really effective on plague if survivors are afraid to infect themselves. I don't know about 4th, I could go with shadowborn just to see better, use whatever.

As for addons, I usually like to just go with some basic faster puke with either decreased cooldown or maybe longer corrupt purge. If the survivors seem toxic, I might switch to that ultra rare addon that lets me see them whenever they vomit so it's easier to track them.

Remember to fully vomit downed survivors. :)

I run Corrupt Intervention, Infectious Fright, PGTW, and Thrilling Tremors. I sometimes switch out Thrilling Tremors for Thantophoia or Discordance depending on what I see in the lobby it's my flex slot.

All of her teachables are actually very good on her. Particularly Infectious Fright. Plague also has the most synergy of any killer with Thanatophobia. Blood Echo, while not considered a great perk in general, does have some interesting synergy with Plague as well.

And of course, any perks that work well on most killers (BBQ, Surveillance, Whispers, PGTW, etc.) will work equally well on Plague, with the exception of anything healing-related (such as Nurse's Calling) since healing is quite rare against Plague if you're playing her right, and anything that's based on hitting survivors (like STBFL) because Plague typically only gets one hit per chase.

Also, here are some tips for encouraging survivors to cleanse, from a thread by @DudeDelicious, who is an experienced Plague main (sorry for tagging you again mate, just wanted to give credit where it's due):

    • Plague MUST get EVERYONE infected as early in the match as possible. Let survivors PASSIVELY become broken if necessary. If in a difficult chase and other survivors are not yet infected, infect vaults and pallets during the chase and FORCE who you’re chasing to use them before abandoning chase and find other survivors to infect.
    • When everyone is broken well before or by mid game, not cleansing becomes a much riskier survivor strategy.
    • Even if NOT using DD and/or IF, when survivors are broken and ARE NOT cleansing then SLUG and quickly seek out someone else to punish in their one hit state. Make them regret not using those pools.

I personally run Thanatophobia, Blood Echo, Infectious Fright, and BBQ.

The first three all force the survivor's hand into giving you your power if they want the ability to work on gens faster and have their exhaustion perks available. BBQ because bloodpoints and if you're on a really big map knowing they're far away helps. Sometimes I'll switch in Stridor if I think they're being sneaky.
