So I tried getting the evil incarnate achievement with the tuft of hair and the tombstone piece, however after killing the first survivor I was put into t2 and kicked out of perma t3. So I am wondering if this is a new bug or has it always been the case?

This has always been the case since Tombstone Piece reduces your Evil Within back to T2. Try it with Judith's Tombstone instead.

Have to use the full tombstone if you want to do it. It's going to be a bit of a nightmare to pull off though.

Run whispers + Nemesis + PWYF + Discordance, add ondms tuft of hair + judiths tombstone
From tgere its just stalking everyone till you got t3. Took me 2 games, just got it this morning. Then i ran scratched mirror on gideon to celebrate 😊
