Its actually a 0% chance of being represented in the game.
If we are talking about the "Killer(s)" from the trilogy. Impossible. They don't outright kill characters in any specific or consistent ways.
If you are taking about the actual human centipedes? Even more impossible since they are a group of people stitched together and not some killer entity.

Do you mean the actual "human centipede" or the Doctor / Security guy / Prison boss?
It would not make sense to use the Human Centipede as Killer because these are actually the victims enjoying sexy moments defecating in each others mouths. And the others would be kinda unspectacular so not too sure what to think of it....

Do you know what "The Human Centipede" is? Before you watch it; it's 3+ people stitched together (butt to mouth) and that's the human centipede. They're actual humans just a little bit ...reassembled.
