Why does Dead Hard not work when it says I've exhausted the perk? (PS4)

Numerous times I've encountered this problem while using the perk.
I have killer behind me, I see the red stain in striking distance, I press X right on time and WHAM!
Down I go. Fine, maybe I was too late.
No, it says Exhausted: Dead Hard. I did it correctly, thus I get NOTHING. No benefit of the perk well used.
This has happened countless times, about 7 times today after playing a handful of matches.
It's like using a styptic agent while being chased. You have a 50/50 chance of it working when it says you used it.
If this perk is considered meta then so is Left Behind!
