Xbox - i can't hook survivors

With every killer, i start the game normally, i play normally too, nothing special happen, the only thing is different, is the fact that i can't hook survivors some random times, i try hook them, they escape, i down they again, and magically, in the second try i can hook, i a lot of time was thinking that someone was behind me, but, when someone is behind you, the option to hook don't appears, so this is really a bug, and don't have a standard, is completely random.
