What’s the best way to play as Doctor on a big outdoor map?

Had a few games recently where I’ve struggled like hell against rank 1 survivors and red rank SWF teams on some of the larger outdoor maps. I’m not sure on how best to handle them, especially when they gen rush and are spaced out.
Im running BBQ, Ruin, Undying and then either distressing, franklins or make your choice.
On the more balanced maps I usually end up with merciless killer and max points, on these trickier games I’m getting one or two kills and have had to resort to dirtier tactics just to get that!
ive tried running tinkerer but that just stresses me out more and the gen usually pops by the time I get there anyway.
any suggestions would be greatly appreciated.
