Safe hook bug?

Using NM/BT/Deliverance. Run in for a save, get the guy off the hook with BT and then go down immediately after the save. The guy I saved didn't even use the BT, he ran off and the killer took my hook and lost him, and he healed. I never got a "safe unhook" prompt, never got my Deliverance, I even lost emblem points. I've done this a million times before only I was using Deliverance and it's always been a safe unhook according to the emblems.
