This is one of the reasons I play like 2 killer matches a week. You get punished for playing well.

When the killer finds me within the first 30 seconds of the match starting. I swear, it happens all of the time. I sometimes hide or wait until someone else gets hit first, but I feel lame doing that so I usually bolt out into the middle of the map and start working on the middle generator.
When survivors repair the outer generators before the inner ones.
When survivors are overly altruistic.
When survivors suicide because they were the first ones downed, or were tunneled into their second hook phase.
When a survivor will be on a generator 24m away while I'm hooked, and will finish the generator before coming to save me.

Survivors switching last second with all toolboxes. Then a map offering for haddonfield or midwich. One or more has obsession, and several of them have unbreakable, ds, soul guard/dead hard/iron will
I’m a low rank killer btw and see this often

Facing any of the Chainsaw bros.
as a killer seeing Tea bagging survivors loosing and then seeing they've got every second chance perk they can use.

Dead hard for distance instead of evading 1 hit is what makes me ugh.

Starting a match and seeing 3 teammates go down less than a minute in and knowing you're next lol

Someone being pulled out of a locker 20 secs into the match

At the end of a game that didn't go so well (either as killer or survivor) and just seeing a notification on Xbox and seeing that someone has messaged me. Just makes me think 'what have I done now?'

Oh, how I hated that. Luckily I only main one survivor so after P3ing her, I don't see it anymore.

Killers that slug for the 4K
Killers that tunnel/camp at 4 or 5 gen (IMO, after that, all bets are tough)
Survivors that self care all the time with no sense of how the game is going
Survivors that kill themselves simply because of map/killer selection (I find the player controlling the killer can make any killer decent/toxic)
Survivors/killers that play this game like it is an esport and care too deeply about ranks. Red rank come naturally. Playing this game too seriously slowly sucks all fun out of it.

Most recent thing that frustrates me is when I get the same 3 killers over and over when there is 22 of them in a row. Also can't forget idiot teammate and fat shaming spots

Surprise and Madness skillchecks, the game tends to lag on them 10/5 times so you having to deal with regular skillchecks can be a pain, but then you make the success area smaller or the killer uses oppression or you're facing a doctor and it becomes obnoxious solely on the fact that the game chugs slow as hell.

For me, its when the killer slugs for the 4k. Like for real, do you really need to drag the game out for another 10-20 minute for just one extra kill?!
Plus the endgame collapse exists for crying out loud. You can close the hatch! You could have earned way more points by letting the game finish either way and then moving on to your next game than just continuing slugging and then looking for the last guy with no way to know where he is and just hoping you come across him!

No one the last one. If the person on the gen is that close and he saves you it will only get the killer to come back sooner and the gen won’t be fixed.

Loading into a game and realizing it's Huntress. What should be one of the most enjoyable killers to play against is ruined by some of the worst hitboxes ever created.

For me, the big “UGH” comes when they slug for 4K. And then re-slug. At that point I just drop controller (PS4) and grab phone.

Rank 1 killers tunneling and slugging for 4k because there's no obsession.

I can't hate the killer for this. I agree it's an ugh moment when it happens, but the fault is on the devs for not willing to take risks and changing some of the core game play. If a killer did well and outplayed the survivors, they deserve the 4k.
It becomes a big "ugh" moment if the killer was just toxic and sweaty from the start. Then I become a bush and make the game take as long as possible. I won't go for the rescue and make sure I'm near a locker. I rather wait for the bleed out timer to finish and die to the entity than let a sweaty tunneling freddy or spirit get me.

All survivors rushing a newly hooked survivor, facecamping, people not working on gens while killer is obviously occupied, toolboxes being almost null at this point UNLESS you have a team running them, certain synergy killer perks, nurses calling, bbq, etc. I could go on...

When you’ve been running the killer around for a while and just when you’re about to receive your first hit the last gen is fixed, activating NOED. I swear, the timing is sometimes uncanny.
