I prefer Thanataphobia + Dying Light + Hex Ruin + BBQ but this is also a great alternative.

That's not that bad, if they don't heal this build doesn't really do that much other than Thana, and as a Legion you don't have a power basically if everyone is injured.
If you want to torture Survivors, try Ruin + Undying, Thana and Dying Light, if you don't want to use Ruin + Undying, try Pop and Surge.
EDIT: Also use double mend time add-ons.

I feel like it would work on only indoor/very small maps. I've seen a decent amount of people use it and in my experience it doesn't seem very reliable especially if the survivors are good and just don't heal cuz at that point you're an M1 killer with possibly 4 stacks of thana.

I would definitely be rid of Distressing. It gives more bloodpoints, but I don't believe it can go past the cap, and the whole idea appears to max your points in a super long game.
Try Ruin or Dying Light. Or throw out Coulro or BBQ and get both.

Replace BBQ and Distressing for Pop and Surge. Then throw on a Mangled FF addon. NOW we're talking.

I think that any survivor who heals against such build deserves to lose. Just treat it like plague and do basic M1 loops for the win.

I feel like everyone is missing the point of this build LMAO, maybe they don't understand what Iridescent Button does (that's the one that makes your terror radius map wide in Feral, right?) and that Coulrophobia is linked to terror radius (thus making Distressing super useful)
Honestly, though? I'd take Distressing off and put on Tinkerer. That makes for times where they COULD and SHOULD heal, since they won't be affected by Coulrophobia, so it's not perfectly ideal, but it makes survivors WANT to heal rather than stay injured since they will have trouble knowing if they're going to get caught by surprise. But if you want the full gimmick, definitely don't take Distressing off LOL
Most survivor groups you'll get won't be good, anyways, so you'll probably be fine with running your OG build idea without any different perks
