Yes following lore the entity is feeding him his deepest desires and his is to be watched and glorified so he knows (or rather thinks) he is being watched by something.
Plus his insanity probably disillusioned to something like this where he just perpetually thinks he is the centre of attention

Yeah in his lore it states that hes aware of being watched by an audience, and thats actually his reasoning for willingly joining the entitys realm. He chose to join with the promise of an everlasting audience to always be giving him attention.

He is winking at The Entity, his only fan now and its not a teenager full of hormones and birds on the brain but a rather harsh fan, I dont think it will be pleased with winks alone...

I think he does. His lore calls out "the million eyes that would watch him, run from him, experience him" as an incentive
