I am bad at tracking survivors
I easily lose sight of them/get confused and they're just gone. Aside from perks or headphones or settings, what can I do to get better at not losing them?

Play survivor. You’ll get to know how they move better

Really it just takes time in game. You could try using Wraith, with all seeing blood and when cloaked, survivors within 12 meters of you are revealed. This may help you learn survivor moments and how to track. He also has a great perk, Blood Hound, that will help you see blood stains.

If you play on PC and have an NVIDIA graphics card that supports FreeStyle, you can use GeForce Experience to make the DBD environment easier to see. There are video guides on YouTube on how to do this.
Also DBD is supported by NVIDIA themselves so it should be fine.

There are some videos from streamers including Otz on the subject. Some tips:
- Play survivor, you'll get the feeling of how they think.
- Play doctor/legion for easy tracking abilities.
- Lower the quality settings so survivors are easier to spot.
- If you're playing nurse, Shadowborn can increase your FoV while stunned. Do not grow reliant on it tho.

Ok I am gonna write as short as possible and write it one by one.
1 - Look for blood not the marks. Blood is easier to track the survivor when you injure them.
2- When you hit survivors they are immune to your body collision for a short time and survivor with Iron Will can hide inside your body. So move a little bit and check your surrondings.
3- Crows can be quiet helpful and use them to track survivors. Most of the survivors will try to lose line of sight when they escape so check the animals and tight corners.
4-Sometimes after you hit them due to your camera and like I mentioned at the number 2 due to body immunity they just crouch after you hit them. Because of your camera you thought they did run but actually they were just there.
5- Noises can be your best bet. Listen to their groanings to detect where they are.
6- When you hit them instead of moving forward either move one step back or stay where you are for 1-2 secs to detect them what are they gonna do.
