I think if the Entity were to have a physical form, considering what it's based on, I don't think the Entity would have such a humanoid form.

I also don't think it'd work gameplay wise. She controls this realm. Her power is quite limitless and can do whatever she wants whenever she wants

In the original French, I believe they used feminine pronouns when referring to the Entity. In English, I think it is only referred to as It.
The Entity is what you want her to be.
