    Is this quote from his head cosmetic what you're talking about?

    If so, I think it's meant to mean that Pinhead is the leader of demonic creatures (Cenobites) that are devoted to experimental torture AND that they are also devoted to the ruler of hell, Leviathan. Not that he is the ruler of hell himself. It's probably just an error in translation, BHVR does this a lot. In reality, it should probably be:

    "The leader of demonic creatures devoted to experimental torture and to the ruler of Hell, an extra-dimensional realm."
