Surviors don't pick up the puzzle box first thing
I just got out of a match in which one of the survivors picked up the puzzle box right off and started trying to solve it. The killer found him quickly and put him in a deadly state before he could solve it. Since the killer had the box now all the other survivors became revealed and a chain hunt began making the first part of the match disastrous.
Seriously don't pick it the box too early.

I would say so. Two survivors ended up hooked due to it. The one with the puzzle box and a second one while the other two worked to fix the mess. Not one generator got done during that time and the survivor that picked up the box was sacrificed. At that point no one really wanted to rescue them after the fiasco they caused. Ended with only two generators getting done and only one survivor escaped due to the killer letting them go.

Lucky one. You have mates who pick up the box. Getting chased and no one touch the box triggered me always to a self sacrifice...

This! My teammates usually won't tip it, lol. I usually have to solve it myself even if I'm injured... I hate going against him now.

I thought the box disappeared if Pinny teleported to the box? Or did the survivor just hold onto it the whole time and never attempted to solve it before Pinny showed up?

It sounds like the killer found and downed the survivor without teleporting, could be hoarder or just good luck (or an oblivious survivor running around wildly)
