Why some addons that use up medkit dont stack?

Anti-hemorrhagic syringe and refined serum dont stack only the first addon gets used.
Syringe and styptic stack even tho you cant get both of them to work at the same time you either get healed or get endurance.
i dont know if styptic and refined serum stack.
