Balance

It is true that the Devs are afraid of buffing killer perks because of how powerful Nurse is and how these perk changes would make her overpowered ?

I would not say that it is true, but this is one reason why the game will never be completely balanced. Too many factors.

You cannot just buff a Perk (or Killers in general), because design failures like Nurse exist. Something might lift up Killers like Clown or Trapper, but as long as those are Buffs to all Killers or to a Perk, Nurse (and other strong Killers) can also utilize it.

This is why most balance changes involve Tweaks to the actual Killers (their power or Add-Ons) and not the whole Killer-Side.

New Ruin is the perfect example.

It made high pressure mobility killers even stronger but did nothing for low mobility m1 killers (the ones who actually need slowdown)

Even shrinking the maps will just make killers like Blight or Nurse more oppressive. Less pallets help Pig but create more dead zones for Blight to get easy downs.

Killers have never had more stackable game delay than they have now

Doubtful, there was a conspiracy that they wouldn't fix Nurse's bugs because of how powerful she was, and yet, they took the time to do that on this update that was mostly regarding the health of the game over balance or content.

In the case of perks, people ignore that 90% of them are nearly useless. However, that obviously indicates that the good perks are the exception and not necessarily what they strive for. Clearly they intend for perks to be minor enhancements to playstyle rather than meta defining. I think they are moving away from that attitude, slowly but surely.
