If I already have the perks unlocked at level 45, are the tier 3 perks unlocked forever

Only you get tier 3 perks on everyone if your character is tier 3

I assume you mean if you have the teachables unlocked at 30, 35 and 40?
Peanits said in another thread that they'll unlock at Tier 1 on other characters that don't have them yet:
To get them to Tier 3 automatically on characters that don't have them yet, you'll need to get the character to P3.

To get the perk on all other characters immediately post-update
Yellow perks of that character for all other characters requires you to have 70-119 Perk points.
Green perks of that character for all other characters requires you to have 120-169 Perks points.
Purple perks of that character for all other characters requires you to have 170-219 Perks points.
Each perk your character owns is worth points based on the rarity of the perk.
Yellow perk = 1 point. Green = 2 points. Purple = 3 points.

Not sure exactly what you are asking, but any perks you have already unlocked on a particular character will stay unlocked on that character at the same tier after the update.

You'll have their perks at tier 1 automatically on all characters. Unless you already have them at higher tiers on certain characters, which will stay the same.
As for characters you've leveled further but have not prestiged. If you have 4 pages of purple perks on a character, they'll be P3 automatically, and therefore provide their perks to all other characters at tier 3 automatically.

You get the tier 1 perks of a character on the other characters if you bought the teachables, so you don't need 70 perk points reaching level 40 and unlocking the teachable is enough.
