Backwater Swamp isn't Spawning Hatch

Platform: PC (probably affects all)
Description: The hatch on Backwater Swamp (particularly the Grim Pantry) isn't spawning in many circumstances.
Reproducing: Just spawn on the map enough times and the hatch will just not spawn, period.
Frequency: Somewhat Often
