I don't get this. Please explain to me like I'm 5 Years Old

What exactly is the point of this? If it consumes the med-kit on use, then how can you possibly finish healing yourself or your teammate?
You lose a whole med-kit for 5 seconds of endurance? A fellow teammate gave me a med-kit with this and I didn't know what to do.

It's a meat-shield button. Killer comes along while you're healing someone, you use it and run away.

You can’t finish a heal after using it, that’s the point of it consuming the medkit. But you can use up most of the medkit charges and then use this addon later as long you have just a tiny bit of charges left. If you want to ensure you have enough to self-heal once and then still be able to use this addon, pair it with Bandages so you’ll still have 8 charges after the self-heal.
You can either use it on a teammate if you can’t finish healing them in time before the killer comes, or you can use it on yourself mid-chase. It’s like Dead Hard but better since you can fast vault with it and the killer can’t down you (just don’t vault a pallet with it or slow/medium vault a window because then the killer can grab you and that negates the Endurance).

If I bring in a med-kit with just that add-on, can I use the purple agent if my med-kit is empty? As in like I already healed myself once.

No, ‘cause an empty medkit cannot be used.
you need to be healing (as in, holding M2, or your equivalent of that) to receieve the prompt to use it.

Yes, it is very strong if used properly. It actually used to be even stronger, it got nerfed a few months ago (from 8 seconds to 5 seconds).

It's a free DH without an animation. You can get a full self heal with a med kit and then use the leftover charges plus this addon to get an extra health state the next time you get chased

Throw on a single charge addon (like the Bandages) and you'll have enough for a heal + enough charges to use the Styptic and then some. You don't even need them if you hit Great skill checks consistently, as they save you some charges.
Either bring one of the addons or hit a couple of greats and you have a one-heal medkit with a single-use super Dead Hard. It's a really good addon.

It's not as good as a syringe or a medkit that can give two heals, but after that its solid.
You don't want to use it with a full medkit, but it does make something out of having only a few charges left. I'll try to use it in chase, after I make a corner of a loop pop it, let the killer catch and hit me, and then run. It's not going to save you by any means, but it can extend the chase which is pretty useful.

Can I use it with this? In one medkit? This heals a Survivor without your hands on their back, right?

You can, both effects will trigger at the same time. However, if you get deep wounded from the killer hitting you while the Styptic Agent is active, when the syringe timer is up it will only remove the deep wound and you’ll still be injured. But yes, it is possible to use them together.

As Raconteurminator stated, if the killer finds you mid heal, you can pop the styptic to gain endurance for five seconds and if the killer attacks you within those five seconds, you will tank the hit and won't go down. This will leave you in deep wounds and will completely consume your medkit.

If I use these add-ons on a teammate in SoloQ, how will they know that I used it on them?

I'm not sure. I don't usually bring items into the trial with me, so the only time I used one was when I found one on the ground. I didn't know what it did, so I used it on a teammate thinking it would heal them. Instead I just wasted the medkit and the survivor looked unimpressed.
It might give them an endurance icon on their screen whilst it is active, but I couldn't say for certain.

It's an incredibly strong item. Grab any medkit, put the agent addon on, put a +8 charges addon on.
Now you have a full heal medkit, and then have just enough spare charges left over to have an on demand endurance effect. Killer comes up, just activate the agent with your last 8 charges, and weeee, you get another free health state.

For the styptic it will show them the Endurance status effect on the right, for the syringe there will be a blue icon with its symbol at the bottom of the screen which also shows how much longer before its heal will trigger, the time varies since it is affected by heal speed modifiers.

Anything that affects healing speed, including perks, the Mangled status effect, and the medkit itself. The syringe takes the same amount of time to heal as it would for you to complete that heal normally from start to finish. So using it on a teammate will actually be faster than 16 seconds (unless there is something else in play that reduces the heal speed) since medkits are faster for healing teammates, but it will actually be slower if you use it on yourself since medkits reduce self-heal speed.

originally all item add-ons that had an on-use effect consumed their respective items, mind you originally it meant only the syringe, the septic and the brand new part. Back then using a brand new part just instantly finished the gen (even from 0 progress).
They have reworked brand new part through a few iterations that included not to consume the associated toolbox, and they have never touched medkit addons in this way. Maybe its long overdue.
