You're in luck, while it isn't this exact game, there's another post with screenshots of the mechanic failing to do it's job:
Interesting to me, how people want to hand wave and dismiss in this post, but there's a suspicious silence when someone actually posts pictures of the killer next to the hook.

He is to close on the first and third screenshot. Progress bar doesn´t fill, when survivors are literally next to the hook. To prevent survivors abusing the mechanic, by looping the killer around the hook.
Could also be caused by the Bot Meg. She isn´t visible on the screenshots, but since bots behave in special ways, she´s probably close by.
