Is Desperate Escape (achievement) bugged?

I got 3 hits in one match and got 0 charges fast vaulting in less than 1 second. Escaped a match where I never got hit and got 1 charge a few matches before that. How does this achievement actually activate? The only time I consistently get a charge is if I'm meleed as I'm in the middle of a fast vault.
