Action buttons not doing what the screen says

Can someone explain to me why this game so frequently gets this wrong? I can be standing next to a window and on the screen I see "R1 Vault". But when I press R1 my player will instead throw a pallet that's near or go in a locker. Of course it's not just with is a vault. The screen can show if I press R1 I'll be healing but when I press it instead I vault.
Is it too much to ask for the action listed on screen to be the action they actually takes place?
