The horse belongs to the Clown (it pulls the Clown's cart). I believe its name is Maurice, and as stated above, the Entity basically gave it a third eye.

Going off the clowns bottles it looks like while creating them he had an accident an Maurice was caught in cross fire an had his face melted an the reason it's there is cause it goes with the gypsy wagon as it would have pulled it to that point before the clown untied it so it can lay next to the wagon 😉0

I hate Maurice. Its sound gives me the creeps. And as killer, I often mistake its movement in the edge of my field of view for some pesky survivor.

His name's Maruice. The clown was so evil, that the entity let him take the horse into the realm. Maruice was blind, so the entity gave it an eye.1

Yeah like other's said, the horse is named Maurice and he belongs to the clown, it's implied the entity allowed Kenneth to bring his horse along because of how evil of a person he is, the entity just gifted him a third eye to makeup for his blindness.
