DEFINITELY M&A for getting closer. As if you can catch them off guard it might be an instant down, and that's massive
Honestly it's hard to think of a definitive 4th needed perk. Maybe Iron Grasp or Agitation? New Thanatophobia? mostly personal preference

Monitor & Abuse is excellent on her, almost core perk. Gives you enough time to get close to survivors and vomit on them otherwise they would just preemptively run away.
For the 4th one it's really up to you, i'd honestly go for spirit fury since pallets are what hinders her for the most part. You can also consider Play with your food since it doesn't count for Vile Purge but only for Corrupt Purge.
Other meta things like Nurse's calling, Thanato, Sloppy and etc. are, suprisingly, a big no-no on her. Simply because of the instaheal at fountains... if it wasn't for that, Thanato would be almost OP in her hands.

Monitor and Abuse is apparently super good on her. It's a Perk I def want on her.
As for a second Perk I'd guess these being decent:
Surveillance
Make Your Choice
Play With Your Food
Hex: Third Seal
Spirit Fury
Enduring (for DS changes)

I imagine Discordance would be a handy dandy perk. If her purge affects Franklin’s, that would be mercilessly hilarious.

Monitor & Abuse - for getting closer! Core perk!
Corrupt Intervention - Allows you to patrol the gen a lot easier!
Bamboozle - Stops the window loop effectively!
Brutal Strength - Some pallet loops hinder her the most part! So you need to break them ASAP!
Enduring+Spirit Fury - Same with Brutal Strength but much better, and it grants you a free hit!

Honestly, I would go with Infectious Fright or Tinkerer.
Tinkerer actually works amazingly on her to let you jump the generator folks and get a quick down.
Infectious Fright just helps you escalate your one down into two or more downs.

Haunted Grounds works well with Ruin and Thana might be worth running now the numbers are buffed.

I'd consider forgoing Ruin in favor of Corrupt Intervention and coupling it with Discordance. The combo should help spread sickness to survivors in the early-game, and the fact you'll have a few corrupted fountains around afterwards means you don't fall behind when Corrupt Intervention wears off. Discordance will also keep benefiting you the whole game, as when you find multiple survivors together you can take advantage by infecting one and chasing after the other.
However, if you want to stay with the classics and use Hex: Ruin, I feel you're best off running detection perks in your last slots, such as Whispers or, well, Discordance. Nurse's Calling will probably be quite weak on her, along with Thanatophobia and especially Sloppy Butcher, given how infected survivors can quickly heal themselves at fountains. So if you want to run another game delay perk, your only real option is Pop Goes the Weasel, which admittedly shouldn't be bad on her. Aside from that, Brutal Strength is good on chases, and the best end-game perk will probably be Blood Warden, given how her power doesn't synergise well with NOED nor Remember Me.

I would use Infectious Fright and Knockout, so lets say you chase the first person and down them, if you hear someone nearby you go after them slugging the previous Survivor. From there you take down this 2nd Survivor and hook them for BBQ, if no one has saved the 1st Survivor hook them and then you create massive pressure with Ruin being active and 2 hooked Survivors.
When both Survivors get unhooked you'll get 2 Corrupted fountains and from here you can play off of this always making sure you have at least 1 fountain available at all times, slugging when needed.

For 4th slot, maybe it's really up to me or pick whatever i want.

Does the Blindness effect the fountain auras? Cause i know it doesnt effect rhe pig's boxes.

They worked just like The Pig's boxes, so I don't think Blindness would work

I don't think anti-heal perks will be too good on Plague considering that the fountains insta-heal survivors. Stuff like Sloppy, Thana, Nurse's, etc will all end up being somewhat worthless.
People say M&A is good on her, I can understand why but eh... I think stuff like Tinkerer or Dark Devotion will be better.
Enduring/Spirit Fury will be really strong for her since she's still weak to loops. Brutal Strength also just to eat through pallets faster.
BBQ/Ruin is good on every killer, nothing special for Plague.
I think Blood Warden might be a good perk for her. If you get a proc and have corrupt vomit you can basically multi-down in the gates.
Knockout would be good for her since it can slow the game and spread the sickness (when they heal them up). Plus because of Broken they won't be able to heal all the way.
STBFL would be good for her since you can avoid losing tokens by inflicting Broken with the vomit.
Surveillance should be good for her too since you can kick a gen, puke on it, and if they tap it they both alert you and get sick.

Discordance is great cause you can find multiple people to yarf on at once.

I don't think Tinkerer and Dark Devotion will be better than Monitor & Abuse, even though decreases TR more than M & A, as they have time limits, and The Plague doesn't have high-mobility, unless if they're close to the survivors!
but yeah, they still work! And any anti-heal perks didn't work quite well on her due to the fountain!
