Escaping the Trial
Whenever a survivor escapes the match, what if:
(By Hatch Only) They still have the Reverse Bear Trap on them
The Plague's Sickness
Legion's Deep Wound (Actually, that can be healed by other campfire survivors easily, no problem.)
Doctor's Madness
Still in Dream World (Well, other Campfire Survivors can wake you up, easy as that)
But that would mean that the Plague can just spread the sickness easily through every survivor outside of the trial maps, and survivors affected by the Doctor will just keep on seeing hallucinations. How would survivors get out of these outside of the trials?

maybe the entity heals each survivor before each trial, that would also explain why they just teleport into the trial and have no injuries at all, besides no mither, as the entity would transport them, and while doing so heal all wounds, injuries, sickness, etc, and then if they are running no mither, punish the survivor with broken... just a thought

Interesting... I still don't get No Mither in a lore standpoint. What type of trauma did David have to go through? :O

Considering the Entity feeds on Hope it would make sense he tries to fill each survivor as much as possible before sending them into the feeding grounds.
