Depends on how many are dead,
Hatch will now always spawn and open no matter how many gens are done when 3 survivors have been sacrificed/killed
The hatch will APPEAR when:
There is 2 generators left and 2 survivors are dead
There is 1 generator left and 1 survivor is dead
No generators left and all survivors are alive

Pretty sure the accepted formula is number of survivors left + 1 (so if survivors left are 2, then 3 gens must be completed before it will spawn i.e. have 2 gens remaining) also as above if there is only one survivor left (regardless of gen number in this case)
Pretty good to remember especially for trophies such as where did they go? In which case all generators must be completed before the hatch spawns with all survivors alive.
