If you're not injured go in a locker and get grabbed out with ds, when you get free are you at a injured state or still healthy?

Injured because the killer had you in their grasp

The moment you're grabbed (or as it's called, "Interrupted") by the killer you immediately enter the Dying state actually, not merely injured. Look at your status portrait next time you get grabbed. So by the time you actually hit DS health-wise it's no different to if they picked you up off the ground.

Anytime you get grabbed and break free you'll be injured.

I get your point but getting into a locker is (and should be) a risk which is only up to you to take.
